Dog
looked OK after surgery. A large
E-collar to be worn. Wait 7-14 days.
Daily nursing. Atropine given. If
things go well, the eyesight will be
saved due to the relatively prompt
attention from the owner as advised by
her groomer in Bishan. If only
treatment was sought 3 days when the
eye was painful, there would be no
need for this surgery and worry about
blindness.
Illustration is best to
explain what had happened. I don't permit the Shih Tzu to go
home after surgery as many owners do not know how to nurse the
dog. Give an advisory letter if the owner insists on bringing
the dog home.

horizontal mattress 2/0 absorbable
sutures stitch up eyelids to protect
the corneal ulcers and give them time
to heal without the irritation of
breezes, light and dust. A
Subconjunctival antibiotic injection
was given. Antibiotic eye ointment
given.
Xylazine 2% IM. After 10
minutes, mask onto the nose to knock down with 5% isoflurane.
Then intubate. Isoflurane gas maintained at 1.5%. This is an
old dog. Be careful. Uneventful anaesthesia.
Descemet's membrane (basement layer of
the cornea) had protruded out from the
ulcer and seal the leak. However blood
from injured iris had shot out earlier
(lots of bleeding seen by the owner).
Free blood clots can be seen in the
anterior chamber. These may cause
glaucoma or disappear after a few days
if there is no infection. If there is
infection, the eyeball turns
yellowish.
Eye thoroughly flushed to
get rid of the hairs and debri. Pull out the 3rd eyelid to
flush any trapped dirt. I saw a ripple in the cornea
indicating collapse at 3 to 6 o'clock as aqueous from the
anterior chamber leaked out. The eye did not collapse. I can
see 2 ulcers. The deep corneal ulcer with the Descemet's
membrane at the centre and another bigger concentric ulcer
caused by the Shih Tzu's eye rubbing. Can you see them in the
picture 2406 below?

2412. A very serious eye injury in a
7-year-old Shih Tzu in Singapore.
Two corneal ulcers (one is deep
and one is superficial),
blood in the anterior chamber, leakage
of aqueous from the anterior chamber
and descemetocoele. The dog is likely to be
blind due to delays in seeking eye
treatment. Eye injuries have
better prognosis when treated
within 24 hours after injury. This
case was at least 3 days later and
bacterial infection would have
gone inside the eye. Hope is all
we have.

- 2403. Veterinary anaesthesia is more
important in this case than in being
the most skilful surgeon. The vet or
his assistant must know how to
handle emergencies in
cardiac arrest during anaesthesia.
Before the dog dies. A
dead dog is never appreciated by the
owner. This surgery buys time for
the old dog. I do not want to do a
second anaesthesia and surgery as
the dog is likely to die. The
large tumour removed had grown
explosively only during the last 6
months.
Get your vet to remove very small
tumours rather than wait and wait
till the tumours are inoperable or
the dog is in grave danger.

- 2399. Instantaneous Decisions. I passed by
Plaza Singapore. The
young ladies had posed for the first
photographer friend (my left). I clicked
the young ones of Singapore. Two
girls had made instantaneous
decision to make eye contact with
me. Eye contact makes a picture
interesting to the viewer because
eyes of the posers "communicate"
with the viewer.
Using music
like this anti-smoking rock concert is effective in
attracting the attention of the young
ones to the ill effects of smoking.
Yet there are young ones
smoking beside the stage.
